    Error message on form

    I keep getting the following error message on a form I have created:

    website builder
    Warning: Cannot modify header information - headers already sent by (output started at /home/tjboskvm/public_html/christmas_meal.php:2) in/home/tjboskvm/public_html/christmas_meal.php on line 82


    The form can be found here: http:www.mowcopchurch.com/christmas_meal.php
    Can anyone tell me what I am doing wrong?

    You have added the following code:

    <METAHTTP-EQUIV="Pragma"CONTENT="no-cache">

    in the Page HTML (in Start of Page)

    Please remove it.
