Inventory not decreasing after purchase

After an item is purchased its quantity is not being removed from inventory. The number of items I have remains the same. This option �When customers add this product to their cart, require Form Data from:� is set to per quantity. I have tried it both ways. Current version I am running is 4.9v56.1. This did work at one time.

I just completed more testing and it only fails to remove the inventory when the payment option is check or money. If the payment is credit card or Paypal the inventory decreases.
