    Default Form not working

    I've made a hundred of them before, but for some reason im having problems with this form.

    Visit the site and you'll see what I mean.

    What am I doing wrong? Think im just having a bad day.

    Delete the page with the form
    then republish the page as .php
    (page properties - file extension)
