Default Forms from Wizard - Need help

Ive already read the tutorials explaining how to design forms from Wizard in BlueVoda
I created one of these forms, step by step, as is showing in the tutorial and it seems easy. The form has only 3 fields: Name, email, Comments and a Submit buttom. I can see the form in the previw page. Then, I published my page but after that I dont see the form in the published page!
This is because the program is a beta and these forms does not work or am I missing something else?

Tx David. That was it. It works

But..now I have another little problem
    I have a frame inside de index.html
When I submit a comment opens the success.html page. Ok with that
In the success.html page I have a link to return to the index.html page.
The problem now is that the whole page opens inside the frame: an index.html inside the frame of the index.html page.
If I change the address to initial_page.html of course it does not open the index.html page.
How do I find a solution for this?

Can you check it please? My page is www.hospitaldahorta.org
    Tx again
